New Patient Registration

Thornbrook.pngThornbrook extends a warm welcome to any new patients from Chapel-en-le-Frith,  Chinley, Buxworth, Fernilee, Whaley Bridge, Dove Holes, Peak Dale, Peak Forest, Wormhill, Kettleshume and part of Furness Vale. We offer a couple of ways in which to register with the practice.  Thornbrook is not taking part in the new optional NHS plan to allow patients to register at the practice, regardless of where they live.  To register at Thornbrook you are most welcome if you live in the areas listed above.


To register you will need to complete and return a registration form - these forms can be collected from reception or downloaded from the above "Registering Online" tab.


The form asks for routine details (e.g. name, address) and also some basic medical history (allergies, family history of illness, vaccinations).


We invite all new patients joining the practice to make an appointment, when they register, for a new patient registration check. It may take several months for your full medical records to be sent to us and attending this check makes sure we know important information about you eg. repeat medication, allergies, immunisation status and current medical problems.



Parents with children under 5 years of age are asked to please notify the health visitor at Chapel Health Centre on 01298 812552.
